Duties and Responsibilities:

  1. Maintain and keep the line of communication open with hospital social workers, discharge planners, families and other referring agencies.
  2. Manage the inquiry/referral process professionally, timely and with the appropriate follow-up.
  3. Verify insurance coverage. Maintain a good knowledge of Medicare, Medicaid, Managed Care and Insurances.
  4. Collects and verifies accurate clinical, payor, and social information from the patient, family/responsible party, discharge planner, physician, or other community resources to coordinate a smooth resident admission.
  5. Conduct tours of the facility to perspective residents, family members and marketing clients.
  6. Obtain appropriate admission papers and signatures from residents or responsible parties.

Job Requirements:

Purpose of Your Job Position
The primary purpose of your job position is to supervise the day to day nursing activities of the facility during your tour of duty. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing, to ensure that the highest degree of quality care is maintained at all times.

Essential Functions of the Job

Supervisory Responsibilities

1. Assist in developing, implementing, and maintaining nursing objectives and standards and departmental policies/procedures. (Examples: resident care plans, job descriptions, nursing procedures, etc.)

2. Make independent decisions regarding nursing care.

3. Evaluate emergencies and determine emergency measures to be used; implement measure and inform nursing administration.

4. Assume the authority, responsibility, and accountability of directing nursing care, and delegate appropriately to other responsible personnel.

5. Make timely rounds, inspecting the nursing service areas and practices, to assure compliance with current applicable policies and regulations.

6. Check resident’s condition. If there are any abnormal conditions, contact the doctor for orders. Coordinate admissions to the hospital if necessary.

7. Interpret department policies/procedures to staff, residents, visitors, and family members as necessary and insure that facility policies are followed at all times.

8. Guide and direct the charge nurse in evaluating and assessing medical, social, psychological, spiritual, restorative, and rehabilitative care plans.

9. Assist and instruct the charge nurse and other nursing personnel in performing nursing procedures as necessary.

10. Reviews and analyzes shift quality assurance reports, accident/incident reports and medication error reports. Conducts thorough investigations and takes appropriate corrective action.

11. Create and maintain an atmosphere of personal interest and positive reinforcement, as well as a calm environment throughout the units.

12. Assure that nursing documentation is informative and descriptive of the nursing care provided and of the resident’s response to care.

13. Maintain confidentiality of all pertinent resident care and employee information to assure that resident and employee rights are protected.

14. Administer the Volunteer Feeding Program.

15. Evaluate residents to determine if they are eligible for Hospice care. Contact Hospice and make arrangements for residents that are eligible.

16. Prepare written assignments for employees with special needs.

17. Prepare counseling statements in conjunction with employee evaluations. Administer disciplinary measures as necessary.

Recordkeeping Responsibilities

1. Review complaints/grievances and make necessary oral and/or written reports to the appropriate nursing administration.

2. Assist in evaluating and implementing recommendations from the facility’s committees and consultants.

3. Perform administrative requirements such as completing necessary forms, reports, etc., and submit to the appropriate nursing administration as required.

4. Develop and maintain nursing schedules as necessary. Schedule outside appointment coverage, and administer the transfer sheet.

5. Offer input on recruitment, selection, and training of competent department personnel and make recommendations to the Director of Nursing as appropriate.

6. Offer suggestions for programs designed for inservice education, on-the-job training, and orientation classes for newly assigned department personnel. Monitor inservice attendance and insure that staff are offered all necessary education in order to meet regulations and assure that the highest degree of quality resident care is afforded the residents.

7. Participate and assist in departmental studies and projects as assigned or that may become necessary.

8. Complete job performance evaluations of same shift nurses, and review job performance evaluations of other same shift nursing staff.

9. Prepare Resident Skin Assessment Report on a quarterly basis for Administrator and Department of Health.

Other Responsibilities

1. Develop and maintain a good working rapport with interdepartmental personnel as well as other departments within the facility to assure that services and activities can be properly maintained to meet the needs of the residents.

2. Meet with department personnel in identifying and correcting problem areas, and/or improvement of services.

3. Meet with nursing administration, medical, and other related departments in planning and coordinating department services, programs, and activities.

4. Review, evaluate, and recommend equipment and supply needs, as well as work force status, on a timely basis to nursing administration.

5. Attend and participate in professional activities as well as continuing education, seminars, and programs, etc., to assure that awareness of current regulations and guidelines, as well as professional standards, and make recommendations on changes in policies and procedures to nursing administration.

6. Be accessible to staff, residents, and families and in meeting their needs.

7. Contact residents’ families to report incidents, changes in condition, hospitalization, or death. Meet with residents’ families at other times as necessary.

8. Be cognizant of budgetary parameters and show efforts to operate on shift within those parameters.

9. Attend charge nurse and supervisory meetings. Attend committee meetings as requested or required, as necessary.

10. Exercise principles of infection control in all aspects of work.

11. Observe personnel policies.

12. Perform other job-related duties as required.

Education
Must be a licensed as a registered nurse in the Commonwealth of Pennsylvania, and a graduate from an accredited school of nursing. Additional job-related on-going education required.

Experience
Must have a minimum of four (4) years nursing experience; one (1) year minimum in long-term care and one (1) year supervisory experience.

1.Participate in medical care evaluation studies for Quality Assurance.

Purpose of Your Job Position: The primary purpose of your job position is to assist in the day‑to‑day accounting functions of the facility in accordance with current acceptable accounting principles relating to the long‑term care operation, and as may be directed by the Business Office Manager.

Duties and Responsibilities:

  1. Responsible for tracking residents’ income and their accounts in our computerized system.
  2. Take all income checks and put in order.
  3. Post Social Security, Pensions, and other income for residents to computer.  Deposit these checks to the appropriate accounts.
  4. Post all income and charges to and from residents’ accounts.
  5. Write checks and deduct funds from residents’ personal accounts what is due for maintenance and adjustments.
  6. Answer relatives and/or residents’ questions either in person or by phone about residents’ accounts or checks.
  7. Distribute Quarterly Financial Statements to residents and families.
  8. Disburse residents’ funds as necessary and keep an adequate amount of money on hand to do this.
  9. Make end-of-month bank deposits (e.g., Gift Shop, residents, etc.)
  10. Communicate with families and outside agencies concerning errors or questions on bank accounts, pensions, etc.
  11. Maintain a file of copies of all debits, credits, etc., issued to each resident.
  12. Assist in balancing accounts receivable.
  13. Prepare and mail statements.
  14. Make written and oral reports/recommendations to the Office Manager concerning accounting functions.
  15. Perform secretarial functions as necessary or directed.

Essential Functions of the Job

Maintenance Duties
1. Inspect, lubricate, dismantle, clean, repair, replace, install, maintain, and assemble mechanical equipment.
2 .Operate simple tools such as drill press, pipe threader, power saw, electric welder, lawn mower, and others.
3. Maintain logs of equipment or functions such as the performance of boilers and emergency generators.
4.Care for grounds and garden areas by mowing grass, planting, watering, pulling weeds, trimming shrubs, collecting leaves, and shoveling/plowing snow from driveways, parking lots, and walkways; maintain driveway (snow removal and salting, leaf raking, roadway repair, etc.).
5.Patrol grounds and buildings on a periodic basis to prevent fire, theft, vandalism, and illegal entry.
6.Check interior and exterior lighting for repair or replacement as necessary; perform such repair, replacement, or arrange for such repair.
7.Collect garbage daily throughout the building; clean trash cans and dumpsters (when weather permits); arrange for collection of isolation waste (bio waste): collect and store until can be disposed of properly.
8.Rebuild, repair, and refinish walls, furniture, and fixtures.
9.Supply oxygen tanks to nursing units.
10.Supervise and/or participate in fire or other emergency drills, including assisting at drill sites or evacuating residents as may be necessary.
11.Transport residents for appointments and pick up or drop off items as may be needed, accompanied by nursing personnel where necessary.
12.Perform minor plumbing (leaky faucets, leaking pipes) or electrical repair (replacing plug ends, call bell ends).
13.Monitor areas such as crawl-space and roof for leaks and cracks; make minor repairs when necessary.
14. Perform duties related to maintaining pest control.
15.Wax, buff, and/or strip floors as may be needed.
16.Report areas where work or repair is needed to Superintendent.
17.Determine the optimum time to shut down equipment for repair to minimize damage and loss of work production.
18.Replace filters as necessary throughout the facility.
19.Paint parking lot lines and curbs.
20.Perform unscheduled maintenance tasks as directed.
Other Responsibilities
1.Attend annual or special inservice programs as requested or required.

Cedar Haven Healthcare Center, a 324-bed skilled nursing facility, provides high quality superior skilled nursing, short-term rehab, memory care, and hospice services in Lebanon. We are seeking dedicated and caring employees to join the experienced team in our therapeutic recreation department at Cedar Haven!

Hours of Work:  Hours are flexible, including alternating weekends, Holidays and evenings as needed

The TR Aide assists by encouraging residents to participate in activities that are reflective of their previous interests. They will assist with daily programming, lunch in the main dining room, and with large groups off the unit. They will provide one-on-one visits with residents and are responsible to complete MDS 3.0 on a weekly basis. The ideal candidate would be creative and able to plan, adapt, and implement projects and activities to the residents’ abilities during group sessions such as:

-Arts and Crafts,
-Horticulture
-Music
-Pet Therapy
-Literary Activities
-Food prep
-Card/Board Games
-Physical Activity
-Spiritual Events

The ideal candidate must be able to:
(1) relate to the residents’ life experiences and current interests,
(2) monitor and chart residents’ activity involvement accurately,
(3) work independently and
(4) possess patience and an understanding of the residents’ concerns and needs.
