LPNs

Cedar Haven Healthcare Center, a 324-bed skilled nursing facility, provides superior skilled nursing, short-term rehab, memory care, and hospice services in Lebanon. In 2016, Cedar Haven celebrated 50 years of providing high-quality healthcare for the Lebanon community thanks to our skilled and compassionate team. We are seeking dedicated and caring LPNs (Licensed Practical Nurses)to join the team in this deficiency-free facility!

LPN Responsibilities:

  • Supervise and direct the day-to-day functions of the medication nurse, and/or nursing aides.
  • Supervise all resident/patient treatments.
  • Assist physicians and the Director of Nursing in the treatment and examination of patients.
  • Perform administrative duties such as documenting nurses’ notes, physicians’ orders, discharge plans, dietary changes, reports, evaluations and studies.
  • Conduct and monitor prescribed treatments, such as laboratory work, etc. as assigned.
  • Recognize, intervene and report complaints and grievances from patients, families, visitors and facility.
  • Encourage an atmosphere of optimism, warmth and interest in patients’ personal and healthcare needs.

Qualifications:

  • Education: Licensed Practical Nursing License from an accredited nursing program, college or university.
  • Relevant Experience: Minimum of six (6) months experience in a hospital, long-term care facility or other healthcare institution.
  • Required license or certification: Licensed Practical Nurse

Hours: Part time 7a-3p, 3p-11p or 11p-7a; Full time 3p-11p; or Full time 11p-7a

RNACs

Cedar Haven Healthcare Center, a 324-bed skilled nursing facility, provides superior skilled nursing, short-term rehab, memory care, and hospice services in Lebanon. In 2016, Cedar Haven celebrated 50 years of providing high-quality healthcare for the Lebanon community thanks to our skilled and compassionate team.

We are seeking full-time RNACs (Registered Nurse Assessment Coordinators) for Monday through Friday positions at our deficiency-free facility. The primary purpose of this position is to assist in the achievement and maintenance of high quality, cost effective resident care. This is accomplished by medical record review to establish appropriateness and effectiveness of services, and to ensure these services are being delivered in the appropriate environments, in accordance with current applicable policies and regulations. Participate in medical care evaluation studies for Quality Assurance, evaluate identified changes in condition and initiate subsequent actions. Our RNACs coordinate and conduct a full comprehensive assessment with quarterly reviews on all residents. This includes a review of medical and psychosocial history as well as interviews with the resident, families and staff.

Cedar Haven provides paid training for CNAs through our Nursing Assistant Training Program. Our current class is underway and our next NAT class has not yet been scheduled.
