LPNs

LPN Responsibilities:

  • Supervise and direct the day-to-day functions of the medication nurse, and/or nursing aides.
  • Supervise all resident/patient treatments.
  • Assist physicians and the Director of Nursing in the treatment and examination of patients.
  • Perform administrative duties such as documenting nurses’ notes, physicians’ orders, discharge plans, dietary changes, reports, evaluations and studies.
  • Conduct and monitor prescribed treatments, such as laboratory work, etc. as assigned.

Nursing Supervisor

Hours of Work
-PT 11:00 p.m. to 7 a.m.
-FT 6:30 a.m. to 3:00 p.m., 2:30 p.m. to 11:00 p.m., or 10:30 p.m. to 7:00 a.m. and other hours as volunteered or required, with two 15-minute rest periods and ½ unpaid lunch period per shift, and report time as needed.

Purpose of Your Job Position
The primary purpose of your job position is to supervise the day to day nursing activities of the facility during your tour of duty. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing, to ensure that the highest degree of quality care is maintained at all times.

Reporting Relationships
Responsible to: Director of Nursing, Assistant Director of Nursing
Employees Supervised: All same shift nursing employees.

Essential Functions of the Job

Supervisory Responsibilities

1. Assist in developing, implementing, and maintaining nursing objectives and standards and departmental policies/procedures. (Examples: resident care plans, job descriptions, nursing procedures, etc.)

2. Make independent decisions regarding nursing care.

3. Evaluate emergencies and determine emergency measures to be used; implement measure and inform nursing administration.

4. Assume the authority, responsibility, and accountability of directing nursing care, and delegate appropriately to other responsible personnel.

5. Make timely rounds, inspecting the nursing service areas and practices, to assure compliance with current applicable policies and regulations.

6. Check resident’s condition. If there are any abnormal conditions, contact the doctor for orders. Coordinate admissions to the hospital if necessary.

7. Interpret department policies/procedures to staff, residents, visitors, and family members as necessary and insure that facility policies are followed at all times.

8. Guide and direct the charge nurse in evaluating and assessing medical, social, psychological, spiritual, restorative, and rehabilitative care plans.

9. Assist and instruct the charge nurse and other nursing personnel in performing nursing procedures as necessary.

10. Reviews and analyzes shift quality assurance reports, accident/incident reports and medication error reports. Conducts thorough investigations and takes appropriate corrective action.

11. Create and maintain an atmosphere of personal interest and positive reinforcement, as well as a calm environment throughout the units.

12. Assure that nursing documentation is informative and descriptive of the nursing care provided and of the resident’s response to care.

13. Maintain confidentiality of all pertinent resident care and employee information to assure that resident and employee rights are protected.

14. Administer the Volunteer Feeding Program.

15. Evaluate residents to determine if they are eligible for Hospice care. Contact Hospice and make arrangements for residents that are eligible.

16. Prepare written assignments for employees with special needs.

17. Prepare counseling statements in conjunction with employee evaluations. Administer disciplinary measures as necessary.

Recordkeeping Responsibilities

1. Review complaints/grievances and make necessary oral and/or written reports to the appropriate nursing administration.

2. Assist in evaluating and implementing recommendations from the facility’s committees and consultants.

3. Perform administrative requirements such as completing necessary forms, reports, etc., and submit to the appropriate nursing administration as required.

4. Develop and maintain nursing schedules as necessary. Schedule outside appointment coverage, and administer the transfer sheet.

5. Offer input on recruitment, selection, and training of competent department personnel and make recommendations to the Director of Nursing as appropriate.

6. Offer suggestions for programs designed for inservice education, on-the-job training, and orientation classes for newly assigned department personnel. Monitor inservice attendance and insure that staff are offered all necessary education in order to meet regulations and assure that the highest degree of quality resident care is afforded the residents.

7. Participate and assist in departmental studies and projects as assigned or that may become necessary.

8. Complete job performance evaluations of same shift nurses, and review job performance evaluations of other same shift nursing staff.

9. Prepare Resident Skin Assessment Report on a quarterly basis for Administrator and Department of Health.

Other Responsibilities

1. Develop and maintain a good working rapport with interdepartmental personnel as well as other departments within the facility to assure that services and activities can be properly maintained to meet the needs of the residents.

2. Meet with department personnel in identifying and correcting problem areas, and/or improvement of services.

3. Meet with nursing administration, medical, and other related departments in planning and coordinating department services, programs, and activities.

4. Review, evaluate, and recommend equipment and supply needs, as well as work force status, on a timely basis to nursing administration.

5. Attend and participate in professional activities as well as continuing education, seminars, and programs, etc., to assure that awareness of current regulations and guidelines, as well as professional standards, and make recommendations on changes in policies and procedures to nursing administration.

6. Be accessible to staff, residents, and families and in meeting their needs.

7. Contact residents’ families to report incidents, changes in condition, hospitalization, or death. Meet with residents’ families at other times as necessary.

8. Be cognizant of budgetary parameters and show efforts to operate on shift within those parameters.

9. Attend charge nurse and supervisory meetings. Attend committee meetings as requested or required, as necessary.

10. Exercise principles of infection control in all aspects of work.

11. Observe personnel policies.

12. Perform other job-related duties as required.

Comply with Department and Cedar Haven fire prevention and safety standards, including the reporting of equipment malfunctions and hazardous conditions.

1. Maintain the confidentiality of all resident care information and observe the principles of geriatric care.

Education
Must be a licensed as a registered nurse in the Commonwealth of Pennsylvania, and a graduate from an accredited school of nursing. Additional job-related on-going education required.

Experience
Must have a minimum of four (4) years nursing experience; one (1) year minimum in long-term care and one (1) year supervisory experience.

Hours of Work: 8:00 a.m. to 4:00 p.m.; 4:00 p.m. to 12:00 a.m.; 12:00 a.m. to 8:00 a.m.; and other hours as volunteered or required, with two 15-minute rest periods and one 30-minute unpaid meal period per 8 hour shift.

Purpose of Your Job Position: The primary purpose of your job position is to maintain the grounds, facility, equipment in a safe and efficient manner in accordance with current applicable federal, state and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by your supervisor, to assure that a successful maintenance program is maintained at all times.

Reporting Relationships: Responsible to: Maintenance Supervisor. Employees Supervised: None.

Essential Functions of the Job

Maintenance Duties

1. Inspect, lubricate, dismantle, clean, repair, replace, install, maintain, and assemble mechanical equipment.

2 .Operate simple tools such as drill press, pipe threader, power saw, electric welder, lawn mower, and others.

3. Maintain logs of equipment or functions such as the performance of boilers and emergency generators.

4.Care for grounds and garden areas by mowing grass, planting, watering, pulling weeds, trimming shrubs, collecting leaves, and shoveling/plowing snow from driveways, parking lots, and walkways; maintain driveway (snow removal and salting, leaf raking, roadway repair, etc.).

5.Patrol grounds and buildings on a periodic basis to prevent fire, theft, vandalism, and illegal entry.

6.Check interior and exterior lighting for repair or replacement as necessary; perform such repair, replacement, or arrange for such repair.

7.Collect garbage daily throughout the building; clean trash cans and dumpsters (when weather permits); arrange for collection of isolation waste (bio waste): collect and store until can be disposed of properly.

8.Rebuild, repair, and refinish walls, furniture, and fixtures.

9.Supply oxygen tanks to nursing units.

10.Supervise and/or participate in fire or other emergency drills, including assisting at drill sites or evacuating residents as may be necessary.

11.Transport residents for appointments and pick up or drop off items as may be needed, accompanied by nursing personnel where necessary.

12.Perform minor plumbing (leaky faucets, leaking pipes) or electrical repair (replacing plug ends, call bell ends).

13.Monitor areas such as crawl-space and roof for leaks and cracks; make minor repairs when necessary.

14. Perform duties related to maintaining pest control.

15.Wax, buff, and/or strip floors as may be needed.

16.Report areas where work or repair is needed to Superintendent.

17.Determine the optimum time to shut down equipment for repair to minimize damage and loss of work production.

18.Replace filters as necessary throughout the facility.

19.Paint parking lot lines and curbs.

20.Perform unscheduled maintenance tasks as directed.

Other Responsibilities

1.Attend annual or special inservice programs as requested or required.

2.Comply with department and Cedar Haven fire prevention and safety standards, including the reporting of equipment malfunctions.

Laundry Worker

Hours of Work: 6:00 a.m. to 2:00 p.m. every Saturday and Sunday also other hours as volunteered or required, with two 15-minute rest periods and one 30-minute unpaid meal period per 8 hour shift.

Purpose of Your Job Position: The primary purpose of your job position is to perform the day to day activities of the Laundry Department in accordance with current federal, state and local standards, guidelines, and regulations governing our facility, and as may be directed by the Administrator and/or the Director of Laundry Services, to assure that our facility is maintained in a clean, safe, and sanitary manner, and that an adequate supply of linen is on hand at all times to meet the needs of the residents.

Reporting Relationships: Responsible to: Laundry/Housekeeping Director, Assistant Laundry/Housekeeping Director. Employees Supervised: None.

Essential Functions of the Job

Duties and Responsibilities

1.Picks up bagged soiled linen and resident’s clothing from the nursing units at intervals as directed.

2.Take soiled laundry in designated carts directly to soiled linen room in laundry.

3.Sorts laundry for color, removes non-washable items, weigh loads for washer and records weights on appropriate records.

4.Loads washers with appropriate capacity and type of laundry. Uses appropriate formulas into washer as prepared by the laundry consultant which starts the washers and runs them through the complete cycle.

5.Uses special wash cycles for items that require it.

6.Removes damp, clean laundry from washers into clean cart, and transport to proper area for further processing.

7.Operates washers simultaneously and continuously washing soiled laundry until directed to stop or until all soiled laundry in the department is washed.

8.Transports tumble wash to dryers and loads dryers according to directions, observing temperature and time required for proper drying of each load.

9.Inspects all laundered wash for stains, large tears, or other problems necessitating special handling.

Related Duties

1.Folds items dried in dryers and places like items together in piles that are then placed in clean linen carts to be delivered to clean linen closet.

2.Transports finished laundry to areas where used in amounts and types of laundry items as directed by laundry management.

3.Participates in environmental and equipment cleaning of the laundry as directed.

4.Attend annual or special in-service programs as requested or required.

5.Exercise the principles of infection control in all aspects of work.

6.Observe personnel policies.

7.Follows OSHA safety regulations, including using proper safety and protective equipment.

8.Perform other job related duties as required.

5.Must maintain the care and use of supplies, equipment, etc., and maintain the appearance of laundry areas, for sanitation, order, and safety.

6.Must be willing to work harmoniously with other personnel as well as be willing to handle residents based on whatever maturity level at which they are currently functioning.

7.Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing laundry practices.

8.Must be able to follow written and oral instructions concerning the mixture of cleaning compounds, liquids, cleaning instructions, etc.
