Purpose of Your Job Position
The primary purpose of your job position is to supervise the day to day nursing activities of the facility during your tour of duty. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing, to ensure that the highest degree of quality care is maintained at all times.

Reporting Relationships
Responsible to: Director of Nursing, Assistant Director of Nursing
Employees Supervised: All same shift nursing employees.

Essential Functions of the Job

Supervisory Responsibilities

1. Assist in developing, implementing, and maintaining nursing objectives and standards and departmental policies/procedures. (Examples: resident care plans, job descriptions, nursing procedures, etc.)

2. Make independent decisions regarding nursing care.

3. Evaluate emergencies and determine emergency measures to be used; implement measure and inform nursing administration.

4. Assume the authority, responsibility, and accountability of directing nursing care, and delegate appropriately to other responsible personnel.

5. Make timely rounds, inspecting the nursing service areas and practices, to assure compliance with current applicable policies and regulations.

6. Check resident’s condition. If there are any abnormal conditions, contact the doctor for orders. Coordinate admissions to the hospital if necessary.

7. Interpret department policies/procedures to staff, residents, visitors, and family members as necessary and insure that facility policies are followed at all times.

8. Guide and direct the charge nurse in evaluating and assessing medical, social, psychological, spiritual, restorative, and rehabilitative care plans.

9. Assist and instruct the charge nurse and other nursing personnel in performing nursing procedures as necessary.

10. Reviews and analyzes shift quality assurance reports, accident/incident reports and medication error reports. Conducts thorough investigations and takes appropriate corrective action.

11. Create and maintain an atmosphere of personal interest and positive reinforcement, as well as a calm environment throughout the units.

12. Assure that nursing documentation is informative and descriptive of the nursing care provided and of the resident’s response to care.

13. Maintain confidentiality of all pertinent resident care and employee information to assure that resident and employee rights are protected.

14. Administer the Volunteer Feeding Program.

15. Evaluate residents to determine if they are eligible for Hospice care. Contact Hospice and make arrangements for residents that are eligible.

16. Prepare written assignments for employees with special needs.

17. Prepare counseling statements in conjunction with employee evaluations. Administer disciplinary measures as necessary.

Recordkeeping Responsibilities

1. Review complaints/grievances and make necessary oral and/or written reports to the appropriate nursing administration.

2. Assist in evaluating and implementing recommendations from the facility’s committees and consultants.

3. Perform administrative requirements such as completing necessary forms, reports, etc., and submit to the appropriate nursing administration as required.

4. Develop and maintain nursing schedules as necessary. Schedule outside appointment coverage, and administer the transfer sheet.

5. Offer input on recruitment, selection, and training of competent department personnel and make recommendations to the Director of Nursing as appropriate.

6. Offer suggestions for programs designed for inservice education, on-the-job training, and orientation classes for newly assigned department personnel. Monitor inservice attendance and insure that staff are offered all necessary education in order to meet regulations and assure that the highest degree of quality resident care is afforded the residents.

7. Participate and assist in departmental studies and projects as assigned or that may become necessary.

8. Complete job performance evaluations of same shift nurses, and review job performance evaluations of other same shift nursing staff.

9. Prepare Resident Skin Assessment Report on a quarterly basis for Administrator and Department of Health.

Other Responsibilities

1. Develop and maintain a good working rapport with interdepartmental personnel as well as other departments within the facility to assure that services and activities can be properly maintained to meet the needs of the residents.

2. Meet with department personnel in identifying and correcting problem areas, and/or improvement of services.

3. Meet with nursing administration, medical, and other related departments in planning and coordinating department services, programs, and activities.

4. Review, evaluate, and recommend equipment and supply needs, as well as work force status, on a timely basis to nursing administration.

5. Attend and participate in professional activities as well as continuing education, seminars, and programs, etc., to assure that awareness of current regulations and guidelines, as well as professional standards, and make recommendations on changes in policies and procedures to nursing administration.

6. Be accessible to staff, residents, and families and in meeting their needs.

7. Contact residents’ families to report incidents, changes in condition, hospitalization, or death. Meet with residents’ families at other times as necessary.

8. Be cognizant of budgetary parameters and show efforts to operate on shift within those parameters.

9. Attend charge nurse and supervisory meetings. Attend committee meetings as requested or required, as necessary.

10. Exercise principles of infection control in all aspects of work.

11. Observe personnel policies.

12. Perform other job-related duties as required.

Comply with Department and Cedar Haven fire prevention and safety standards, including the reporting of equipment malfunctions and hazardous conditions.

Resident Rights

1. Maintain the confidentiality of all resident care information and observe the principles of geriatric care.

2. Monitor nursing care to ensure that all residents are treated fairly, and with kindness, dignity, and respect.

Working Conditions

1. Works in office areas as well as throughout the facility.

2. Sits, stands, bends, lifts and moves intermittently during working hours.

3. Is subject to frequent interruptions.

4. Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.

5. Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.

6. Communicates with the medical staff, nursing personnel, and other department supervisors.

7. Works beyond normal working hours and on weekends and holidays when necessary.

8. Is involved in community/civic health matters/projects as appropriate.

9. Attends and participates in continuing educational programs.

10. Is subject to falls, burns from equipment, odors, etc., throughout the work day.

11. Maintains a liaison with the residents, their families, support departments, etc., to adequately plan for the residents’ needs.

Education
Must be a licensed as a registered nurse in the Commonwealth of Pennsylvania, and a graduate from an accredited school of nursing. Additional job-related on-going education required.

Experience
Must have a minimum of four (4) years nursing experience; one (1) year minimum in long-term care and one (1) year supervisory experience.

6. Must have patience, tact, a cheerful disposition, and enthusiasm, as well as the willingness to handle difficult residents.

7. Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.

8. Must be able to relate information concerning a resident’s condition.

9. Must not pose a direct threat to the health or safety of other individuals in the workplace.

Job Title:  Registered Nurse MDS/ICP Coordinator

Purpose of Your Job Position: The primary purpose of your job position is to assist in the achievement and maintenance of high quality, cost-effective resident care.  This is accomplished by medical record review to establish appropriateness and effectiveness of services, and to ensure these services are being delivered in the appropriate environment, in accordance with current applicable policies and laws.

Essential Functions of the Job

Minimum Data Sets/Interdisciplinary Care Plan Coordinator Functions

1.Participate in medical care evaluation studies for Quality Assurance.

2.Evaluate identified changes in condition an initiate subsequent actions.

3.Coordinates and conducts a full comprehensive assessment (MDS) with quarterly reviews on all residents.  This is to include review of the medical an psychosocial history, interviews with the residents, families, and staff, and physical assessment and ensure that all sections of the assessment have been completed.

4.Ensure that periodic reassessment is accomplished promptly after a significant change in the resident’s physical or mental condition.

5.Coordinates and ensure that the resident’s care plan is comprehensive and complete within seven (7) days of MDS.

Essential Functions of the Job

1.Responsible for all residents income and their accounts

2.Take all income checks and put in order.

3. Post Social Security, Pensions, and other income for residents to computer.  Deposit these checks to the appropriate accounts.

4. Post all income and charges to and from residents accounts.

5.Write checks and deduct funds from residents’ personal accounts what is due for maintenance and adjustments.

6.Answer relatives and/or resident’s questions either in person or by phone about residents’ accounts or checks.

7.Distribute Quarterly Financial Statements to residents and families.

8.Disburse residents’ funds as necessary, and keep an adequate amount of money on hand to do this

9.Make end-of-month bank deposits (e.g., Gift Shop, residents, etc.)

10.Communicate with families and outside agencies concerning errors or questions on bank accounts, pensions, etc.

11.Maintain a file of copies of all debits, credits, etc., issued to each resident.

12.Assist in balancing accounts receivable by running tapes, verifying computer printouts, etc.

13.Prepare and mail statements.

14.Make written and oral reports/recommendations to the Office Manager concerning accounting functions.

15.Perform secretarial functions as necessary or directed.

16.Assist in preparing and implementation of changes in our accounting system as necessary or directed.

17.Monitor and collect accounts receivables.  Report delinquent accounts to the office manager.

18.Perform functions of computer/data processor as necessary.

The TR Aide assists by encouraging residents to participate in activities that are reflective of their previous interests. They will assist with daily programming, lunch in the main dining room, and with large groups off the unit. They will provide one-on-one visits with residents and are responsible to complete MDS 3.0 on a weekly basis. The ideal candidate would be creative and able to plan, adapt, and implement projects and activities to the residents’ abilities during group sessions such as:

Laundry Worker

Purpose of Your Job Position: The primary purpose of your job position is to perform the day to day activities of the Laundry Department in accordance with current federal, state and local standards, guidelines, and regulations governing our facility, and as may be directed by the Administrator and/or the Director of Laundry Services, to assure that our facility is maintained in a clean, safe, and sanitary manner, and that an adequate supply of linen is on hand at all times to meet the needs of the residents.

Essential Functions of the Job
Duties and Responsibilities
1.Picks up bagged soiled linen and resident’s clothing from the nursing units at intervals as directed.
2.Take soiled laundry in designated carts directly to soiled linen room in laundry.
3.Sorts laundry for color, removes non-washable items, weigh loads for washer and records weights on appropriate records.
4.Loads washers with appropriate capacity and type of laundry. Uses appropriate formulas into washer as prepared by the laundry consultant which starts the washers and runs them through the complete cycle.
5.Uses special wash cycles for items that require it.
6.Removes damp, clean laundry from washers into clean cart, and transport to proper area for further processing.
7.Operates washers simultaneously and continuously washing soiled laundry until directed to stop or until all soiled laundry in the department is washed.
8.Transports tumble wash to dryers and loads dryers according to directions, observing temperature and time required for proper drying of each load.
9.Inspects all laundered wash for stains, large tears, or other problems necessitating special handling.
Related Duties
1.Folds items dried in dryers and places like items together in piles that are then placed in clean linen carts to be delivered to clean linen closet.
2.Transports finished laundry to areas where used in amounts and types of laundry items as directed by laundry management.
3.Participates in environmental and equipment cleaning of the laundry as directed.
4.Attend annual or special in-service programs as requested or required.
5.Exercise the principles of infection control in all aspects of work.
6.Observe personnel policies.
7.Follows OSHA safety regulations, including using proper safety and protective equipment.
8.Perform other job related duties as required.

10.May be subject to high noise levels, and hot/cold extreme ambient temperatures.

Education
Graduation from high school or GED certificate desirable.

Experience
Knowledge of cleaning principles and experience in same desirable, but not necessary. On-the-job training provided.
